The basics

What is the gut microbiome?

Your gut microbiome is the vast community of bacteria, viruses, fungi and other microbes that live in your digestive tract — mostly in the large intestine. Far from being passengers, they're working partners: breaking down fibre your body can't digest, producing vitamins and short-chain fatty acids, and helping train your immune system.

No two microbiomes are identical. Yours is shaped by your diet, sleep, stress, medication, environment and genetics — and it shifts over time. That's the interesting part: unlike the DNA you were born with, your microbiome can change, which makes it one of the more actionable windows into your health.

Researchers increasingly use the word diversity as shorthand for a resilient gut. A wide, balanced mix of species is generally associated with better health, while a narrowed or imbalanced community — sometimes called dysbiosis — is associated with a range of digestive and metabolic issues.

Embarrassment kills. People put off mentioning a change in their bowel habits because the subject feels awkward — and that delay is exactly what bowel cancer thrives on. So Dr Tang decided to meet the taboo head-on.

Every April, for Bowel Cancer Awareness Month, he leads an annual street campaign known as the Dirty Protest. Volunteers dressed as poo emojis and loo rolls march through UK city centres — from Manchester to Liverpool's John Lewis at Liverpool ONE — handing out treats, holding emoji balloons, and starting the conversations most people avoid.

The humour is the strategy. It's loud, it's hard to ignore, and it gives people permission to ask the questions they've been sitting on. Dr Tang is there in person to talk through signs and symptoms with anyone who stops.

What a microbiome test can — and can't — tell you
